Signal ladder
- LOG — record incident, no bet blocking
- SHADOW — wins held pending review (default for score ≥ 70)
- PAUSE — block new bets when auto-pause enabled and score ≥ 85
- Cross-bet pre-gate blocks complementary bets at score ≥ 85 before operator debit
